Job Summary:

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at the Texas Department of Aging & Disability Services. As a key member of our interdisciplinary team, you will be responsible for providing high-quality nursing care to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral problems.

Key Responsibilities:
  • Conduct comprehensive physical assessments and develop individualized care plans to ensure optimal health outcomes.
  • Collaborate with the interdisciplinary team to coordinate care and services, including medical appointments and treatment plans.
  • Provide education and support to individuals, their families, and staff members to promote health and wellness.
  • Participate in quality assurance reviews and contribute to the development of nursing policies and procedures.
  • Stay up-to-date with the latest nursing best practices and technologies to ensure high-quality care.
Requirements:
  • Current RN licensure in the state of Texas.
  • Experience working with individuals with intellectual and developmental disabilities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of nursing techniques, standards, and procedures.
