Your Role

As a Care Coordinator Specialist, you will play a vital role in delivering exceptional case management services to our clients. Your primary responsibility will be to assess family strengths and weaknesses, identify existing problems, and develop personalized plans of care.

You will work closely with outreach workers, health educators, nutritionists, and other service providers to prevent service duplication and ensure high-quality care. Your expertise in advocacy, referral, and monitoring will help individuals achieve their goals and maximize their ability to live outside an institution.

Key Responsibilities

  • Conduct home visits and office-based services to gather social history and assess family needs
  • Develop and implement plans of care, including advocacy, independent living skills, training, peer support, physical therapy, and cognitive therapy
  • Collaborate with healthcare professionals to coordinate care and prevent service duplication
  • IDentify and procure group services to enhance individual well-being
  • Monitor progress and adjust plans as needed to ensure optimal outcomes

Qualifications

To succeed in this role, you will need:

  • A Bachelor's degree in human services or equivalent experience
  • One year of related work experience and knowledge of case management
  • A valid driver's license, registration, and insurance
  • Current CPR/First Aid Certification
  • Excellent verbal and written communication skills
  • The ability to establish working relationships with individuals served
