Job Summary

We are seeking a highly skilled and compassionate Qualified Professional to join our team at Southeastern Integrated Care LLC. As a key member of our staff, you will play a vital role in providing exceptional care and support to individuals with mental health and developmental disabilities.

Key Responsibilities
  • Develop and implement Person-Centered Plans (PCPs) in collaboration with clients, families, and healthcare professionals.
  • Provide direct and indirect interventions to support clients in achieving their treatment goals.
  • Coordinate activities within established PCPs, ensuring seamless transitions and effective care.
  • Perform case management functions, including linking clients with community resources and services.
  • Assist Team Leads with discharge planning, ensuring a smooth transition back into the community.
  • Participate in a first responder on-call system, providing 24/7 support to clients and their support networks.
  • Represent Southeastern Integrated Care LLC in a professional and positive manner, reflecting our company's mission and values.
Requirements
  • Bachelor's degree in a Human Services field or related field, with 2 years of experience in mental health and developmental disabilities.
  • Or, Bachelor's degree in a non-Human Services field, with 4 years of experience in mental health and developmental disabilities.
  • Or, Registered Nurse (RN) license in North Carolina, with 4 years of experience in mental health and developmental disabilities.
  • Master's degree in a Human Services field, with 1 year of post-graduate experience in mental health and developmental disabilities.
Work Environment

The work environment for this position is primarily in clients' homes and communities, with occasional exposure to domestic animals, pests, and other environmental factors.

Physical Demands

This role requires regular standing, walking, and use of hands to perform essential duties. The employee must be able to lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds.
