About the Role:

The Lexington Country Place is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality care and services to our residents.

Key Responsibilities:

  • Monitor the health and wellbeing of residents and provide leadership support and assistance with resident care and services.
  • Administer medication and communicate with resident support services and families.
  • Maintain clinical quality assurance and lead, demonstrate, and support all elements of wellness.
  • Identify on-going needs and services of residents through the assessment and Personalized Service Plan (PSP) process.
  • Ensure proper follow through and documentation for residents with a change in clinical care needs.
  • Assist with direct resident care as needed.

Requirements:

  • Must be a Licensed Practical Nurse or Registered Nurse.
  • A minimum of one year of experience working with elders or disabled individuals, preferred.
  • Must have demonstrated Leadership capabilities.
  • Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred.
  • Ability to work weekends, evenings, and flexible hours.

Physical Requirements:

  • Stand/walk: 4-6 hours per day.
  • Sit: 2-4 hours per day.
  • Drive: 30-60 minutes, 1-2 times a week.
  • Lift/carry: 40-70 pounds, occasionally.
  • Use hands for repetitive tasks: simple grasping, pushing, and pulling, fine manipulation.
