Licensed Practical Nurse LPN Home Care Specialist

About the Role:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Care team at AdventHealth. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each patient.
Responsibilities:
- Administer nursing care to patients in accordance with physician orders and under the direction of a Registered Nurse.
- Provide individualized nursing care, teach, and supervise patients and their families or caregivers to ensure continuity of care.
- Deliver high-quality nursing care, perform technical nursing skills with a high degree of proficiency, and promote the team approach in the delivery of patient care.
- Execute physician orders and notify RN/Case Manager of necessary updates or changes in condition.
- Complete documentation according to Agency Policy and Regulatory guidelines and perform home visits as ordered.
Requirements:
- Current LPN licensure in the state of Florida.
- Current BCLS certification.
- Valid Driver's License.
- Vocational training in an Accredited School of Licensed Practical Nursing.
Preferred Qualifications:
- Venipuncture skills.
- Two (2) years of Home Health LPN experience.
- Minimum of (1) year of clinical experience in Medical/Surgical nursing.
- IV certification.
