KEY RESPONSIBILITIES
- • Represent Open Arms Care Corporation in a professional and positive manner during community engagements, events, and social gatherings.
• Organize and implement daily activities that cultivate a structured and uplifting environment. As the primary professional interacting with clients, ensure a person-centered, organized, and non-reactive atmosphere that fosters independence.
• Make independent decisions regarding best practices, quality assurance, risk management, and problem-solving, including timely responses to and documentation of serious incidents.
• Exhibit the ability to coach, instruct, and mentor clients in all facets of home management and personal autonomy, actively promoting individual rights and self-advocacy.
• Engage clients in meaningful ongoing treatment and the individual support plans (ISP) for those served in the ID/DD program; execute programs and deliver services in accordance with the ISP and CMS regulations.
• Advocate for each client's unique needs and well-being in both home and community settings, ensuring follow-up on identified concerns.
• Assist in coordinating, scheduling, and/or transporting clients to appointments and recreational activities regularly; ensure all necessary documents, files, and reports are prepared, reviewed, approved, and processed promptly.
• Maintain accurate documentation of client participation and progress in all training objectives/programs aimed at teaching independent living skills; ensure completion of daily assigned tasks.
• Communicate and actively engage in the interdisciplinary process, including but not limited to individual support plans (ISP), behavior management, incident reports, etc.
• Interact positively with clients' family members/conservators and maintain constructive relationships both internally and externally.
• Attend all mandatory training sessions/meetings and keep current on all certifications.
• Provide support and services to all clients across the local operation as needed.
• Transport clients to medical appointments, enrichment centers, and outings as assigned.
• Responsible for managing work schedules; dependability is essential. Monitor and manage personal time off and accurately document hours worked.
• Execute responsibilities and duties in a professional, timely, effective, and efficient manner, including other assigned tasks.
• Participate in additional committees or volunteer opportunities as assigned.
QUALIFICATIONS
Education & Experience:
- High school diploma or GED preferred.
- Must possess a valid driver's license.
- Ability to lift a minimum of 50 pounds repetitively; tasks may include lifting, repositioning, turning, bending, and squatting.
- Strong oral, written, and interpersonal skills to effectively communicate with others.
