2. Perform and document comprehensive functional assessments of residents, adhering to standardized protocols. Evaluate home, vocational, and equipment requirements.
3. Create and execute personalized treatment plans tailored to resident needs, incorporating attainable functional objectives.
4. Follow evaluation, treatment, and documentation standards, maintaining accurate and timely records.
5. Guide Assistants in treatment methodologies and supervise their interactions with residents closely.
6. Record progress notes in resident files after each treatment session.
7. Adjust treatment plans as necessary throughout the therapeutic process.
8. Prepare clear and concise discharge summaries that document the treatment course using a specified format.
9. Introduce residents to program services and treatment protocols relevant to their individual needs, while informing them of any potential risks involved.
10. Collaborate with nursing staff, residents, and families to enhance the utilization of residents' functional capabilities.
Qualifications1. Bachelor’s or Master’s degree in Physical Therapy.
2. Valid license to practice in the relevant state.
