About the Role:

  • We are seeking an experienced Registered Nurse to join our team as a Senior Patient Care Coordinator - PCU. In this role, you will be responsible for coordinating patient care services across multiple sites within the Cincinnati Market.
  • You will work closely with our nursing teams to ensure seamless transitions of care, communicate effectively with patients and families, and provide emotional support during challenging times.
  • Your expertise will enable you to assess patient needs, develop comprehensive care plans, and implement evidence-based interventions to promote optimal outcomes.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Clinical decision-making and intervention planning
  • Effective communication and collaboration with multidisciplinary teams
  • Patient education and counseling
  • Documentation and record-keeping

Requirements:

  • Degree in Nursing (Associate's or Bachelor's)
  • Current RN license in the designated state of employment
  • 1 year of experience as a Registered Nurse in acute care
  • Basic Life Support certification (required)
  • Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications (required)
