Job Overview

We are seeking a highly skilled Registered Nurse to join our team as a High-Quality Patient Care Specialist. In this role, you will work one-on-one with patients to develop personalized care plans and deliver high-quality medical care.

Salary and Benefits

As a High-Quality Patient Care Specialist at At Home Healthcare, you can expect a competitive salary of approximately $90,000 per year, plus comprehensive benefits, including industry-leading pay, flexible schedules, performance bonuses, and no vaccinations required.

Responsibilities
  • Evaluate and assess patient needs to determine their appropriateness for care
  • Develop patient-specific care plans with the appropriate level and frequency of care
  • Communicate with the care team, physician, payors, patients, and families to address care needs and fulfill patient care plans
  • Deliver high-quality skilled care and supervise the care of other care team members
  • Document care delivery daily in our EMR system
Requirements

To be successful in this role, you will need:

  • Successful clearance of health screens as required by state regulations
  • At least one year of experience in caregiving in any setting, including hospitals, nursing homes, assisted living, or home health/private duty
  • Experience with completing Activities of Daily Living: bathing, dressing, cooking, light cleaning, toilet assistance, transfer
  • Ability to communicate effectively with patients, families, and care teams
