Job Overview

We are seeking an experienced and compassionate Nurse Practitioner (APRN) or Physician Assistant to join our dynamic mobile wound care team in Thurston County, WA.

About the Role:

  • Assess and manage various health conditions in adult and geriatric patients in their homes.
  • Collaborate with local organizations on community health initiatives.
  • Provide tailored treatment and education to meet individual patient needs.
  • Work within a multidisciplinary team for holistic patient care.

Key Responsibilities:

  • Manage a caseload of up to 35 patients, approximately 7-9 per day.
  • Engage with patients, families, and healthcare teams to coordinate comprehensive care.
  • Prioritize efficient time management to maintain high-quality patient interactions.
  • Maintain accurate records and communicate effectively with stakeholders.
