Position Summary:

As a key member of Polaris Pharmacy Services, the Consultant Pharmacist plays a vital role in delivering pharmaceutical care to long-term care facilities. This position is designed for professionals who are committed to enhancing the quality of medication management and patient outcomes.

Key Responsibilities:
  • Conduct comprehensive Medication Regimen Reviews (MRR) as mandated by regulations.
  • Prepare detailed reports based on medication reviews and quarterly evaluations of drug utilization trends.
  • Provide actionable recommendations to optimize clinical care and achieve cost savings through formulary compliance.
  • Review and ensure proper documentation of controlled substances and emergency medication kits.
  • Engage in discussions with facility administrators and nursing directors to address pharmacy-related concerns.
  • Identify and recommend therapeutic interchanges to improve patient care.
  • Participate in quarterly Quality Assurance Committee meetings, presenting findings on pharmaceutical services and staff performance.
  • Conduct interim MRRs for residents with short stays or acute changes in condition.
  • Assist with regulatory compliance and provide support during facility surveys.
  • Adhere to all relevant government regulations, including HIPAA compliance.
  • Perform additional duties as assigned based on business needs.

Qualifications:
  • Bachelor's degree in Pharmacy or PharmD with a focus on geriatrics preferred.
  • Current pharmacy consultant license in the relevant state.
  • Strong clinical, organizational, and communication skills.
  • 2-3 years of experience in clinical pharmacy, with a preference for long-term care experience.
  • In-depth knowledge of nursing home regulations and standards.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook).

  • Ability to sit or stand for extended periods.
  • Occasional physical activities such as climbing, kneeling, or lifting up to 50 pounds.
  • Willingness to work flexible hours as needed.
