Job Title: Home Care Nurse Educator

VNS Health is seeking a highly skilled and experienced Registered Nurse (RN) Educator to join our team in Hoboken, New Jersey.

About the Role:

We are looking for a dedicated and passionate nurse educator to develop and implement clinical education programs that foster high-quality care and improve patient outcomes. As a Home Care Nurse Educator, you will be responsible for designing and delivering educational programs for our clinical staff, ensuring they have the knowledge and skills needed to provide exceptional patient care.

Key Responsibilities:
  • Develop and implement clinical education programs that align with VNS Health's strategic goals and objectives.
  • Collaborate with education staff across the enterprise to develop an annual training calendar and ensure a coordinated approach to ongoing programs.
  • Ensure clinical education programs are aligned with the CMS Triple Aim framework and address patient-centered needs.
  • Manage implementation of clinical staff development programs, including orientation, in-services, training, and other learning activities.
  • Review program-specific educational program outcomes and program evaluations to determine areas for improvement and make recommendations for change and/or new programs.
  • Train and/or oversee training of new and current staff in VNS Health/program philosophy, policy, standards, procedures, documentation, models for service delivery, and efficient, productive, cost-effective work processes.
  • Monitor and evaluate performance of newly hired staff and staff referred on for remedial or additional training.
  • Design new and modify existing educational curriculum and training materials to ensure staff has current knowledge and competencies to meet program contractual and regulatory compliance.
  • Conduct educational needs assessment in collaboration with operations staff to identify learning needs among staff.
  • Develop education programs, such as workshops and/or group learning activities, to address performance issues and develop performance improvement plans for tenured employees.
  • Participate in the development of revised programs and/or materials.
  • Communicate ongoing e-learning opportunities and evaluate e-learning usage and effectiveness.
  • Analyze course evaluations to evaluate effectiveness of training sessions and implement improvements.
  • Participate in the development of annual clinical education strategic goals and objectives.
  • Collaborate with Quality department in the development of and implementation of education-related training programs.
Requirements:
  • Current license and registration to practice as a Registered Professional Nurse.
  • CM certification (Certified Care Management) preferred.
  • Bachelor's Degree in Nursing or equivalent work experience required.
  • Master's Degree in Nursing required.
  • Minimum four years' experience in a community-based or long-term home health setting, including at least two years with a primary focus on education and training.
  • Medicare experience strongly preferred.
