- Provide coordination of services between healthcare providers, ensuring seamless care delivery.
- Facilitate access to necessary physical and behavioral health services, addressing social determinants of health.
- Ensure effective communication between all stakeholders, promoting client-centered care.
- Work collaboratively with team members to provide outreach and engagement services to resistant or hard-to-reach clients.
- Provide information and assistance through advocacy and education, helping clients navigate entitlements and community services.
- Arrange transportation and accompany clients to appointments as needed.
- Participate in ongoing trainings to maintain and enhance care management skills.
- Valid NYS ID or NYS driver's license required.
- Must complete necessary training to administer NYS Eligibility Assessment for HCBS service in the UAS system within 60 days of start date.
- Child and Adolescent Needs and Strengths New York (CANS) certification preferred.
- Bachelor's Degree in a human services or related field required.
- Minimum of two years of experience providing direct services to clients with Serious Mental Illness (SMI), developmental disabilities, substance use disorders, and/or chronic medical conditions.
