Job Summary

We are seeking a Registered Nurse (RN) to join our team as a Nurse Care Specialist. In this role, you will provide nursing services, training, and support to address the medical or psychiatric needs of individuals with developmental disabilities, acquired brain injuries, substance abuse issues, and/or emotional or behavioral challenges.

As a Nurse Care Specialist, you will travel between multiple locations to perform your duties and responsibilities, and be accessible to staff at all locations during working hours.

This is a full-time opportunity with a competitive salary of $38.69 per hour.

Responsibilities
  1. Conduct medical assessments of individuals referred to the program and collaborate with intake staff to determine appropriate placement in the community.
  2. Participate in the development and implementation of service plans and supervise the medical components of the plan, in conjunction with physicians.
  3. Review documents and revise all nursing/nursing-related care plans on a regular basis.
  4. Ensure medical records and reports are current and complete, in accordance with regulatory and corporate standards.
  5. Maintain contact with primary care physicians and specialists, and attend doctor visits with mentors and/or individuals receiving services, when directed.
  6. Promote health and wellness for individuals and ensure nursing actions and steps are put in place for prevention.
  7. Administer medications according to established guidelines.
  8. Provide documentation for all medications given and review medication administration records for compliance.
  9. Oversee the management of high-risk medical conditions and private duty nursing services.
  10. Develop training flowcharts specific to each individual receiving services and provide medical skills training to clients, mentors, respite providers, staff, and biological family members, where applicable.
  11. Present medical skills components at pre-service orientation.
  12. Participate as an Individual Health Plan (IHP) or Plan of Care (POC) team member.
  13. Evaluate the medical needs of each individual for initial IHP/POC.
  14. Ensure IHP/POC goals and objectives are medically indicated.
  15. Generate ongoing healthcare protocols for specific medical conditions outlined in the IHP/POC.
  16. Provide consultation and technical assistance to staff and mentors regarding home care issues and operation of durable medical equipment.
  17. May be required to coordinate, organize, and/or assist with household activities such as light housekeeping and meal preparation.
  18. Transport individuals receiving services to planned and/or necessary activities and appointments.
  19. Participate in on-call systems, as needed.
  20. Travel between multiple locations during work hours.
  21. Be available to staff at all locations during working hours.
  22. May be required to be available after hours for consultation.
  23. Actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee, and Administration meetings.
  24. Perform other related duties and activities, as required.
