Job Summary

We are seeking a highly skilled and compassionate Housing Readiness Case Manager to join our team at Openwork. As a key member of our staff, you will play a vital role in supporting youth in their transition to independent living.

Key Responsibilities
  • Housing Assessments: Conduct thorough assessments to evaluate the housing readiness of youth and provide tailored case management and supportive services.
  • Housing Application Support: Assist youth in completing housing applications, navigating the voucher process, and attending housing briefings.
  • Documentation and Submissions: Manage documentation and submissions to local housing authorities for initial housing processes and recertifications.
  • Move-In Support: Support youth during the move-in process at the HAY Center Campus and provide ongoing case management for residents.
  • Collaboration and Communication: Participate in wraparound meetings and collaborate with internal programs, focusing on the In-care Program.
  • Case File Management: Maintain accurate case files and complete required data entry.
  • Event Participation: Engage in HAY Center and Housing Continuum Events as needed.
Qualifications
  • Education: Bachelor's degree in Social Services, Psychology, Social Work, Criminal Justice, or a closely related field.
  • Case Management Experience: Knowledge of case management principles, practices, and documentation.
  • Reliable Transportation: Reliable transportation is required.
  • Certifications and Background Check: Ability to pass a background check and drug screening.
  • Technical Skills: Proficiency with Microsoft Office.
Preferred Experience
  • Public Housing Experience: Experience with public housing and housing subsidies.
  • Transitional Needs: Familiarity with the transitional needs of youth aging out of foster care.
What We Offer
  • Work Schedule: Monday – Friday; potential weekend work to be discussed during the interview.
  • Compensation: $22.46 per hour with weekly pay.
  • Contract Duration: Through May 2025.
