Job Title: Case Manager

The Phoenix Center is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to clients in need, ensuring they receive the necessary resources and services to achieve their goals.

Key Responsibilities:
  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Conduct regular assessments and evaluations to monitor client progress and adjust plans as needed.
  • Collaborate with clients, families, and other stakeholders to ensure seamless communication and coordination of services.
  • Provide crisis intervention, advocacy, and support to clients in times of need.
  • Maintain accurate and up-to-date client records, ensuring compliance with regulatory requirements and agency policies.
  • Participate in team meetings and training sessions to stay informed about best practices and agency initiatives.
Requirements:
  • Bachelor's degree in a related field, such as social work or human services.
  • Active CDCA licensure.
  • Valid driver's license.
  • Ability to work effectively in a team environment and communicate with diverse stakeholders.
