  • Collaborate with community-based services to provide comprehensive support to families.
  • Provide education and support to families on community resources and services.
  • Conduct regular home visits and assessments to monitor progress and identify areas for improvement.
  • Maintain accurate and timely records of family progress and outcomes.
Requirements
  • High school diploma or equivalent; college degree preferred.
  • Relevant experience working with youth and families in the community.
  • Bi-lingual candidate preferred, but not required.
  • Familiarity with community-based services in Delaware.
  • Possession of a valid driver's license and reliable access to a vehicle.
Preferred Qualifications
  • Ability to implement a variety of interventions for families.
  • Ongoing awareness of community resources.
  • Strong verbal and written communication skills.
  • Good computer skills a plus.
  • Strong organizational skills and attention to detail.

Annual salary starting at $40,000.00.
