About the Role

Leon Health is seeking a highly skilled Care Management Supervisor to support the Health Services Department in achieving clinical quality and service objectives. As a key member of our team, you will play a vital role in facilitating the Case Management processes and directing the day-to-day operations of the Case Management team.

Key Responsibilities
  • Support Clinical Operations: Oversee the day-to-day operations of clinical, non-clinical, and administrative staff to ensure seamless delivery of care management services.
  • Monitor Adherence and Performance: Track and monitor adherence to MOC activities, including HRA, individualized care plans, interdisciplinary care teams, and care transitions.
  • Operational Reporting and Task Management: Review and verify accuracy of daily operational reports and distribute tasks to Case Management staff as directed.
  • Phone and Fax Queue Management: Monitor Case Management phone and fax queues on a daily basis to ensure timely and efficient communication.
  • Collaboration and Communication: Work closely with leaders of Utilization Management to address any Care Management or Utilization Management concerns or barriers.
  • Process Improvement: Identify opportunities for process improvement and assist in developing, implementing, and monitoring action plans based on identified Care Management metrics.
Requirements
  • Knowledge and Experience: Possess knowledge of Dual Eligible Special Needs Plans, Centers for Medicare and Medicaid Services (CMS), and Florida Medicaid.
  • Case Management Best Practices: Understand and apply Case Management best practices to ensure high-quality care delivery.
  • Communication and Interpersonal Skills: Demonstrate effective communication, listening, verbal, and written skills to work collaboratively with team members and stakeholders.
  • Technical Skills: Proficient in Microsoft Word, Excel, and Outlook.
  • Work Experience: 2-5 years of experience in Case/Care Management, with a strong background in problem-solving and analytical skills.
Language Skills

Fluent in English and Spanish.
