Job Summary:
We are seeking a skilled Hospice RN Case Manager to join our team at Insight Global. As a Hospice RN Case Manager, you will be responsible for providing high-quality care to patients and their families, while also managing a caseload of 12-15 patients.
Key Responsibilities:
- Develop and implement individualized care plans for patients
- Conduct regular home visits to assess patient needs and provide care
- Collaborate with healthcare teams to ensure seamless care coordination
- Manage patient records and maintain accurate documentation
- Communicate effectively with patients, families, and healthcare providers
Requirements:
- Registered Nurse (RN) licensure in the state of South Carolina
- At least 1 year of clinical experience
- Active driver's license, dependable vehicle, and insurance
- Relevant Hospice or Home Health experience
What We Offer:
- Competitive hourly rate: $75 - $79k/year
- $5,000 sign-on bonus
