Job Title: Licensed Practical Nurse - LPN

We are seeking a dedicated and compassionate Licensed Practical Nurse - LPN to join our team at Consulate Health Care.

You will have the opportunity to collaborate with our healthcare team to implement individualized treatment plans for each assigned resident, ensuring their unique needs are satisfied.

Responsibilities
  • Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
  • Develop and implement individualized care plans for each resident, taking into account their medical history, physical limitations, and personal preferences.
  • Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
  • Supervise and evaluate Certified Nursing Assistants (CNAs) to ensure they are delivering care according to our high standards.
  • Participate in fall-risk and pressure ulcer prevention programs to ensure the highest level of care for our residents.
Requirements

To be considered for this position, you must have:

  • An active license to practice as an LPN.
  • Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred.
