We are seeking a highly skilled and compassionate Social Worker to join our team at USA Health. As a Social Worker, you will play a vital role in providing exceptional patient care and support to our community.
Responsibilities- Abide by and enforce all compliance requirements and policies, performing responsibilities in an ethical manner consistent with the organization's values.
- Enhance professional growth and development through participation in education programs.
- Utilize proper body mechanics when moving equipment/supplies necessary to perform essential functions.
- Review initial patient screens, including social, cultural, psychological, physical, and financial needs, per unit protocol.
- Initiate consults within 24 hours of the first working day in acute care.
- Perform reassessments according to departmental policy.
- Formulate appropriate and realistic discharge plans, working with patients, families, other members of the healthcare team, and available community-based organizations.
- Work closely with Care Coordinators and Utilization Review nurses to coordinate and implement discharge plans.
- Attend and participate in interdisciplinary patient care rounds or conferences.
- Assist outpatients with crisis intervention and referral to community resources as needed.
- Support families and patients dealing with ethical issues, disability, and terminal illness.
- Ensure continuity of patient care by acting as a resource for patient information.
- Provide after-hours on-call services on a rotating basis.
- Communicate and use appropriate customer relation skills with physicians, patients, families, and healthcare team members in person and via telephone.
- Respond to overhead pages and digital pagers.
- Maintain accurate and complete records by documenting assessments, reassessments, and plans in the patient's medical record as dictated by hospital policy.
- Complete departmental activity reports.
- Accept and complete all duties positively and without conflict.
- Cooperate, help others, and improve the performance of the unit.
- Complete all mandatory unit, educational, and hospital requirements.
- Utilize cost-effective practices in performing all aspects of the job.
- Adhere to current Infection Control and Safety Standards.
- Demonstrate the ability to provide competent patient care to the following patient populations: Neonate, Infant-Toddler/Preschool, School Age/Adolescent, Reproductive Age, Post/Peri Menopausal.
- Participate on committees as assigned.
- Participate in performance improvement activities as assigned.
- Master's degree in social work from an accredited institution as approved and accepted by the University of South Alabama.
- Licensure with the State of Alabama as a Social Worker.
