Responsibilities:

  • Conduct thorough psychosocial assessments to identify patients' needs and develop effective care plans
  • Collaborate with the healthcare team to provide holistic care and support to patients
  • Provide education and guidance to patients and their families on managing kidney disease and dialysis treatments
  • Develop and implement strategies to address patients' social and emotional needs
  • Participate in care planning and team review to ensure patients receive comprehensive care

Requirements:

  • Master's Degree in Social Work from an accredited CSWE institution
  • MSW required; LCSW/ACSW preferred (CA); Licensure/LMSW required in states with licensure (TX, TN)
  • One (1) year of work experience in a behavioral or healthcare-related setting, preferred
