- Collaborate with healthcare providers to develop and implement comprehensive care plans.
- Conduct thorough assessments to identify patient needs and develop strategies to address them.
- Facilitate communication between patients, families, and healthcare providers to ensure informed decision-making.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Master's degree in Social Work (MSW) or Bachelor's degree in Social Work (BSW) with 2 years of experience.
- 1 year of experience in Medical Social Work preferred.
- Licensure by Oklahoma State Board of Licensed Social Workers preferred.
- Basic Life Support (BLS) training or re-training required during orientation.
