Job Summary

We are seeking a highly skilled and experienced Case Management Supervisor to join our team. The successful candidate will be responsible for overseeing a team of case managers, providing supervision and guidance, and ensuring the delivery of high-quality case management services to our clients.

Key Responsibilities
  • Supervise and manage a team of case managers to ensure the delivery of high-quality case management services.
  • Provide regular supervision and guidance to case managers to ensure they are meeting their performance goals and delivering excellent service to clients.
  • Develop and implement case management plans and strategies to ensure the best possible outcomes for clients.
  • Collaborate with other departments and stakeholders to ensure seamless service delivery and effective communication.
  • Monitor and evaluate the performance of case managers and make recommendations for improvement.
  • Develop and maintain relationships with community partners and stakeholders to ensure effective service delivery and collaboration.
Requirements
  • Masters degree in social work, psychology, sociology, or a related field.
  • At least 3 years of experience in case management or a related field.
  • Proven track record of success in supervising and managing a team of case managers.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
