Nurse Practitioner

We're seeking a highly skilled Nurse Practitioner to join our clinical operations team at ChenMed. As a key member of our team, you will provide direct patient care in medical centers, nursing homes, skilled nursing facilities, and home settings.
Responsibilities- Assess and manage acute and non-acute clinical problems, performing physical assessments and documenting patient histories.
- Develop and implement patient management plans, providing counseling and education to patients and their families.
- Collaborate with multidisciplinary teams to ensure effective patient care, participating in clinical rounds and conferences.
- Prescribe medication and treatments, referring patients for consultation when necessary.
- Document patient progress and outcomes, maintaining accurate and detailed records.
- ARNP or similar advanced degree in Nursing required.
- Current Nurse Practitioner Certification in the State of practice required.
- Board certification by AANP or ANCC is preferred but may be required for certain States.
- A minimum of 1 year of clinical experience preferred, but willing to consider strong new graduates.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within the first 90 days of employment.
