Job Overview

We are seeking an experienced Adult Blended Case Manager to join our team at Intercommunity Action I. As a key member of our staff, you will play a vital role in providing direct assistance to individuals with behavioral health disorders, ensuring they gain access to critical resources and services.

Key Responsibilities
  • Provide proactive outreach to clients, engaging with them in their homes, schools, and communities to empower and advocate for their needs.
  • Conduct ongoing assessments to identify client strengths and needs, developing personalized goal plans in collaboration with the individual and relevant supports.
  • Facilitate referrals for expanded mental health programming, linking clients with identified resources while obtaining their consent.
  • Serve as a clinical liaison, coordinating care between service providers and systems serving the client, including behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
Requirements
  • A bachelor's degree in a related field or equivalent experience in human services, preferably with adults.
  • A valid Childline, PA Criminal History, and FBI Clearances prior to working with clients.
  • Reliable transportation and a valid driver's license from the state of residence with a minimum of one year of driving experience and a clean driving record.
  • Availability to work some evening hours.
Estimated Salary: $45,000 - $55,000 per annum
