Job Summary:
PruittHealth Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will provide high-quality nursing care to our patients and their families in the comfort of their own homes. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Key Responsibilities:
- Provide palliative nursing care and ongoing support to patients and their families in the home setting.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Carry out pain management and symptom management plans to ensure patient comfort.
- Respond to the physical, mental, social, and emotional needs of terminally ill patients and their families.
- Adhere to all agency, payroll, billing, and documentation policies and procedures.
- Participate in Performance Improvement Program and present inservice programs, team conferences, and staff meetings.
Requirements:
- Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
- Excellent communication and organizational skills.
- Ability to work a flexible schedule and interact with a variety of different age groups.
- Commitment to the Hospice concept and standards.
