About the Role:

PruittHealth Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our team as a Hospice Care Specialist. As a skilled nursing care professional, you will provide palliative care and ongoing support to hospice patients in their homes, adhering to our agency, payroll, billing, and documentation policies and procedures.

Key Responsibilities:

  • Provide skilled nursing care to assigned patients/families as outlined in the plan of care.
  • Participate in the development of the patient/family interdisciplinary care plan.
  • Respond to the physical, mental, social, emotional, and spiritual needs of the terminally ill patient and their families with respect to their age, religious and cultural practices.
  • Make Nursing Services available on an ongoing basis, including regular nursing visits per the patient's plan of care and/or patient's needs and as directed by the RN Case Manager.

Requirements:

  • A thorough understanding of Hospice Regulations (federal, state, and local government).
  • Ability to handle confidential matters.
  • Ability to work a flexible schedule.
  • Commitment to the Hospice concept and standards.
