Job Summary

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Key responsibilities include following the plan of care, providing treatments, and collaborating with the healthcare team to achieve positive patient outcomes.

Key Responsibilities
  • Provide direct patient care as defined in the State Nurse Practice Act.
  • Implement current nursing practice based on comprehensive assessments and care plans.
  • Document patient services accurately and timely to reflect the care plan.
  • Assess and educate patients and their families/caregivers on diagnosis and self-care.
  • Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group.
  • Use equipment and supplies effectively and efficiently.
  • Manage patient pain and symptoms effectively.
  • Evaluate patient response to treatments/medications.
  • Participate in personal and professional growth and development, as well as quality assessment and performance improvement teams.
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience, with community health/hospice or medical/surgical experience preferred.
  • Current licensure as an LPN/LVN in the State.
  • Compliance with accepted professional standards and practice.
  • Excellent verbal and written communication, and organizational skills.
  • The ability to drive and/or have access to transportation for agency-related travel.
  • Possession of current CPR certification.
  • Understanding of the hospice concept and the needs of the terminally ill.
