Felton Institute is seeking a highly skilled Clinical Case Manager to join our Senior Division team. As a key member of our multidisciplinary team, you will be responsible for providing intensive case management services to older adults with severe psychiatric disorders.
Key Responsibilities- Participate in a multidisciplinary team to provide comprehensive care to clients
- Evaluate client needs and develop comprehensive clinical assessments and treatment plans
- Coordinate care among the team and provide individual and group therapy, crisis intervention, case management, and client advocacy
- Maintain accurate and detailed records of client contacts
- Work closely with interdisciplinary team members to ensure seamless care coordination
- Master's degree or higher in Social Work or other Professional Counseling Programs
- Active license with CA BBS, including ASWs, AMFTs, and APCCs
- Minimum of 2 years of case management and mental health treatment for at-risk older adults
- Strong recovery model approach to care and ability to form strong therapeutic relationships with clients from diverse backgrounds
- Excellent communication skills in written and verbal formats
- Ability to travel within SF to meet clients in various community settings
