Job Title: Physical Therapist

Job Summary:

We are seeking a highly skilled and dedicated Physical Therapist to join our team at Summit Care. As a Physical Therapist, you will be responsible for providing high-quality physical therapy services to patients, caregivers, and facilities in accordance with the principles and practices of physical therapy guidelines.

Key Responsibilities:

  • Evaluate patients to determine their current physical status and develop treatment plans that focus on functional and measurable outcomes.
  • Provide physical therapy screening and evaluation to determine patients' current physical status.
  • Develop and implement treatment plans that adhere to professional standards of practice.
  • Oversee and participate in the maintenance of therapy records.
  • Ensure availability of supplies and equipment.
  • Maintain appropriate patient schedule, as per established guidelines.
  • Provide documentation to maintain quality of medical records, as it relates to reimbursement guidelines, accuracy of billing information, and facility requirements.
  • Participate in patient, family, and facility meetings, as appropriate.
  • Follow administrative policies and procedures, including those related to emergency procedures, infectious disease control, and safety.
  • Coordinate and/or participate in training programs for facility staff, residents, families, and rehabilitation staff.
  • Initiate new program development with a focus on functional outcomes to improve the quality of life of facility residents.
  • Participate in continuous quality improvement.

Requirements:

  • Successful completion of an accredited physical therapy program.
  • Valid physical therapy license to practice in the appropriate state(s).
  • Effective written and verbal communication skills.
  • Effective leadership and customer relations skills.
  • Ability to exercise mature judgment with initiative in analyzing and solving problems.

  • While performing the duties of this job, the employee is consistently required to walk, stand, sit, lift equipment, reach with hands and arms, stoop, talk, and hear.
  • Employee must consistently lift and reposition patients.
