The Social Worker/Behavioral Health Consultant is a vital member of the patient's care team, providing brief action-focused interventions to address a wide range of physical health, mental health, and substance use disorders in a fast-paced setting.
Key Responsibilities:- Identify and treat co-occurring physical and behavioral health needs in the medical setting
- Screen, assess, and diagnose psychopathology, including mental health and/or substance use disorders
- Utilize evidence-based methods, such as Motivational Interviewing, Brief Action Planning, and CBT, to address co-occurring mental health, substance use, and physical health care needs
- Make referrals internally and externally as needed, coordinating care between providers and therapists
- Participate in regularly-scheduled huddles and case conferences with the treatment team
- Master of Social Work (LMSW) license or Licensed Professional Counselor (LPC) by the State of Michigan
- Two years of previous mental health and/or substance abuse assessment and treatment experience preferred
- Loan repayment through the NHSC and Michigan State Loan Repayment Program
