Clinical Pharmacist Pennock and Zeeland Hospitals

Maintains accountability for ensuring the safety and efficacy of medication use. Educates and acts as a consultant to patients/family and members of the health care team related to optimal drug therapy and participates in process improvement activities and drug use evaluations.
Key Responsibilities- Interprets and screens medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
- Safely compounds and dispenses medications for administration to patients.
- Evaluates patients, orders appropriate laboratory tests, and interprets serum drug levels utilizing pharmacokinetic principles; provides prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
- Provides appropriate supervision and direction within appropriate skill level to technicians, residents, interns, and other support personnel.
- Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities.
- Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
- Provides education and counseling to patients/family/significant others/members of the health care team in order to optimize outcomes and appropriate use of medications.
- Ensures that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
- Required Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy.
- 2 years of relevant experience hospital pharmacy Preferred.
- American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice Preferred.
- LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required.
- LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required.
- CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required Or CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required.
This is an Every 4th Weekend and Holiday Coverage Position (2 Holidays per Year Rotating) at Zeeland in Addition to 1 Additional Shift Every Pay Period at Zeeland. The remaining FTE will float between Pennock and Zeeland to backfill PTO.
