- POSITION REQUIREMENTS
- Completion of an accredited practical nursing program.
- Current Licensed Practical Nurse (LPN) certification in the State of NH is mandatory.
- Basic Life Support (BLS) certification for Healthcare Providers is essential.

- FUNCTIONAL RESPONSIBILITIES
- Role Objective
- The Inpatient/Procedural Licensed Practical Nurse is tasked with delivering safe, high-quality care to general care patients as part of a collaborative team under the supervision of a Registered Nurse. This role emphasizes patient and family engagement along with effective communication. The LPN adheres to state nursing regulations and organizational policies. Upon successful completion of state-approved training and with delegation from a Registered Nurse, the LPN may administer intravenous medications.
- Performance Expectations
- Documents and provides nursing assessments, interventions, and education to stable patients under the guidance of a Registered Nurse, in alignment with the patient's care plan. Notifies the Registered Nurse of any changes in patient condition or unexpected findings.
- Reviews the care plan and relevant active orders in the patient’s medical record before delivering care. Aids patients with daily living activities, nutrition, and hydration as per orders and care objectives.
- Conducts and records focused nursing assessments for assigned patients, including vital signs, pain assessment, skin and wound evaluations, functional status, fluid balance, and nutritional assessments.
- Administers oral, topical, and inhaled medications. May also administer intravenous medications upon successful completion of approved training and with specific delegation from a Registered Nurse.
- Applies dressings and assistive devices as indicated by assessments or as ordered by the interdisciplinary team.
- Communicates relevant assessment findings and suggestions for improving patient care directly to the interdisciplinary team members and the supervising Registered Nurse.
- Adheres to policies and procedures related to maintaining a safe care environment, taking action to address safety concerns as necessary.
- Follows medication safety protocols, patient identification processes, specimen collection and labeling, documentation of patient information, and maintains dependability in attendance and scheduling.
- Engages in staff meetings, required training, competency assessments, and learning activities in accordance with organizational expectations. Monitors communications independently to meet these requirements.
