**Job Summary:**

We're seeking a skilled and compassionate Registered Nurse to join our team. As a Registered Nurse-Skilled, LTC, Rehab PT, you'll be responsible for supervising the day-to-day nursing activities in our health center, ensuring that new admissions and discharges are processed correctly, and providing direct nursing care as necessary.

**Responsibilities:**

* Ensure that new admissions and discharges are processed correctly
* Consult with residents, family members, and physicians in planning the resident's care, treatment, and rehabilitation
* Provide direct nursing care as necessary
* Oversee and delegate the activities of the nursing staff
* Visit residents to observe and evaluate their physical and emotional status
* Ensure that the healthcare team follows established regulations, standards, and practices
* Participate in QAPI activities, including participation in annual survey preparation and survey process, and maintenance of CMS standards
* May be responsible for covering other positions or job duties as needed
