Home Health Physical Therapist

We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at LHC Group. As a Physical Therapist, you will play a vital role in helping our patients achieve their full potential and regain their independence.
Job Summary:The Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist will develop and implement a treatment plan to improve and enhance the patient's well-being.
Responsibilities:- Evaluate patient's functional status and physical therapy needs
- Assist physician in developing therapy plan of care
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
- Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
- Current Physical Therapy licensure in state of MA
- Current CPR certification required
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
