Key Responsibilities- Develop and implement individualized plans of care that address the unique needs and goals of each patient.
- Conduct thorough evaluations and assessments to identify patient needs and develop effective treatment plans.
- Provide hands-on physical therapy interventions, including exercises, stretches, and other modalities, to help patients achieve optimal mobility and function.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy.
- Current licensure as a Physical Therapist in the state of Massachusetts.
- Minimum of two years of experience as a licensed Physical Therapist, with a strong background in home care and/or acute care.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities.
- Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
This role will require frequent travel to patients' homes within the designated geographic area. The Physical Therapist will be expected to work in a fast-paced, dynamic environment, with a strong focus on patient satisfaction and outcomes.
