### Job Summary
We are seeking a dedicated and compassionate Social Worker to join our team at Springbrook Center in Westbrook, ME. As a Social Services Specialist, you will play a vital role in enriching the lives of our patients/residents by identifying their strengths, social, emotional, and mental health needs, and connecting them with services to meet those needs.
### Key Responsibilities
* Collaborate with our interdisciplinary team to promote and protect patient/resident rights and well-being, including educating staff on the role of social services.
* Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
* Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life.
* Perform patient/resident discharge planning activities, including family/patient education, nursing center documentation, and referrals to appropriate agencies and suppliers.
* Establish relationships and maintain contact with community-based agencies/services that benefit our patient/resident population.
