Job Summary

As a key member of the Hospice of Lake Cumberland team, the Social Worker will provide comprehensive psychosocial support to patients and their families, ensuring their emotional and social needs are met throughout the care process. This role requires a strong understanding of social work principles and practices, as well as excellent communication and interpersonal skills.

Responsibilities
  • Conduct initial psychosocial evaluations to assess patient and family needs
  • Develop and implement goal-directed care plans in collaboration with the interdisciplinary team
  • Provide individual and family counseling services, education, and referrals as needed
  • Assume the role of advocate for patients and families, ensuring their voices are heard and their needs are met
  • Develop and maintain relationships with community agencies and services to promote interagency cooperation and facilitate referrals
  • Document comprehensive psychosocial information clearly and concisely in a timely manner
  • Participate in weekly IDT meetings to discuss caseload issues and contribute to care planning

Bereavement Services
  • Assist the Bereavement Coordinator in coordinating bereavement services for patients and families, as well as the community
  • Complete bereavement referrals within two weeks of patient death and initiate the bereavement plan of care
  • Facilitate community bereavement groups for children and adults as needed
  • Assist with planning and hosting community memorials
  • May provide grief education for the community
  • Assist with planning and facilitation of Hospice bereavement camp for children

Requirements
  • Master's degree in Social Work from an accredited educational institution
  • One year work experience in a healthcare program
  • Valid driver's license and automobile liability insurance
  • Ability to access home and facilities independently

Working Conditions
  • May be required to work in the inpatient unit
  • Participates in on-call schedule to provide 24-hour, 7-day coverage
