Key Responsibilities
Position Overview
The Community Health Physician plays a vital role in diagnosing, managing, and caring for a variety of health-related issues for Suvida Healthcare's patients. This role involves managing patients within a multidisciplinary, value-based care framework, addressing a spectrum of complexities from low to high-risk cases. The physician will lead a collaborative care team and will be responsible for:
  • Providing care focused on outpatient medicine, continuity of care, health maintenance, improving health outcomes, and disease prevention.
  • Leading the care team within the community health center.
  • Collaborating effectively with a multidisciplinary team to ensure comprehensive patient care.
  • Delivering exceptional service and striving for 5-Star care.
  • Maintaining accurate medical histories and records.
  • Referring patients to specialists as necessary.
  • Participating in daily and weekly team huddles to discuss patient care and manage high-risk cases.
  • Influencing departmental strategies and making informed decisions on complex issues.
  • Overseeing Advanced Practice Providers as needed.
  • Reporting to the Medical Director of the community health center.

Qualifications
Knowledge, Skills, and Abilities
  • Minimum of 3 years of experience, preferably in geriatrics.
  • Active, unrestricted medical license in Texas or Arizona.
  • Current on all required immunizations.
  • Understanding of managed care and value-based care models preferred.
  • Strong leadership, customer service, and communication skills.
  • Ability to demonstrate self-confidence and a positive attitude.
  • Excellent teamwork and communication abilities.
  • Proficiency in Microsoft Office Suite.
  • Bilingual/Bicultural (English and Spanish) preferred.

Education and Certification Requirements
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree.
  • Board certification or eligibility in Family or Internal Medicine.
  • Current DEA license.
