Job Description

As a Service Delivery Coach with the Hampton Roads Community Action Program, you will play a vital role in providing social work, mobility case management, and employment services to individuals and families in need. This position requires a strong understanding of the Whole Family Approach model and the ability to assess the needs of clients, coordinate services, and maintain accurate case records.

Key Responsibilities:

  • Conduct home visits and meetings with clients to assess their needs and develop individualized plans.
  • Teach job readiness skills and other basic skills to clients.
  • Facilitate individual and group meetings to support client progress.
  • Develop and implement individualized plans to help clients achieve self-sufficiency.
  • Provide direct transportation services to clients as needed.
  • Connect clients with community resources and services to support their well-being.

Requirements:

  • Associate's degree in a related field or four years of experience in social work, case management, or a related field.
  • Bi-lingual skills (English/Spanish) a plus.
  • Knowledge of statewide reentry services and community providers.
  • Ability to use multiple software systems, including Office 365 and Adobe.

Work Environment:

The Service Delivery Coach will work in a general office environment with occasional exposure to extreme atmospheric conditions. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the job.
