Responsibilities
  • Provide medication management services to patients, ensuring safe and effective utilization of medications tailored to specific medical conditions.
  • Work closely with patients, providers, nurses, and other healthcare professionals to provide access to specialty medications and ensure optimal patient outcomes.
  • Participate in clinical meetings, training staff, and cross-training for other roles as directed.
  • Precept students/residents as directed and participate in Clinical Subcommittees at MGBSP.
  • Document safety events at time of discovery through MGBSP safety event tool and assist in resolution under guidance of the Safety Officer.
Requirements
  • Current State pharmacy license.
  • Experience (2 years) managing patients' specialty medication needs or clinical pharmacist hospital practice experience.
  • Excellent verbal and written skills.
  • Proficiency with Microsoft Office.
  • Commitment to applying individual clinical skills in a collaborative team environment.
