Licensed Practical Nurse

Key Responsibilities:
- Provide skilled nursing services as dictated by the plan of care and document according to policy/procedures.
- Instruct patients/caregivers in various aspects of patient care and disease management, documenting their comprehension and compliance.
- Observe patient response to care/teaching and make recommendations regarding revision of the plan for patient care to the Registered Nurse.
- Provide and document skilled nursing care according to the plan of care/physicians orders, policies/procedures, and standards of care.
- Record and report symptoms, vital signs, and other observations that may be indicative of care plan reactions and changes.
- Follow and teach infection control and safety policies and procedures.
- Participate in ongoing care coordination and maintain positive relationships with referral sources, physicians/other clinicians, patients/families, and other partners.
- Participate with other nurses in taking weekend, night, and holiday calls on a rotating basis, under the direction of a Registered Nurse.
- Continually strive to improve nursing care through participation in inservice activities, staff development activities, and continuing learning.
- Serve on committees as assigned.
- Perform other duties as assigned by the RN/DON.
Requirements:
- TN LPN license - REQUIRED
- 1 year experience - REQUIRED
- Graduate of a state-approved vocational education program - REQUIRED
