As a Hospice RN, you will work in private homes, residential care facilities, nursing centers, and other hospice care environments. Your responsibilities will include conducting comprehensive assessments of patients' physical, emotional, and spiritual needs, developing and implementing individualized Care Plans, and administering medications and treatments as prescribed by physicians.

Responsibilities:
• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
• Develop and implement individualized Care Plans based on patient assessments
• Administer medications and treatments as prescribed by physicians
• Monitor patients' condition and document changes in their health status
• Collaborate with interdisciplinary teams to coordinate patient care
• Provide education and support to patients and their families on end-of-life care options
• Assist with pain management and symptom control
• Offer emotional support to patients and their loved ones
• Attend IDG meeting and trainings
• Must input and complete visit notes timely

Qualifications:
• Must be a Registered Nurse (RN), graduate of an approved school of clinical education and licensed in the state where they currently practice
• Must have at least one year experience as a Registered Nurse
• Must have 1+ year of hospice nursing experience
• Medicare hospice experience is preferred
• Must have basic demonstrated technology skills, including operation of a mobile device
• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
• Must possess CPR certification for the healthcare provider
