Job Summary

We are seeking a part-time/per diem MSW to join our Hospice team in Portage, MI, with our Troy office serving as the parent agency. As a Hospice Social Worker, you will play a vital role in providing psychosocial, emotional, and spiritual support to patients and their families.

Responsibilities
  • Conduct initial assessments and develop goal-directed care plans in collaboration with the interdisciplinary team.
  • Provide care under the direction of a physician, participating in IDT conferences and documenting patient/family visits, telephone contacts, and referral actions.
  • Conduct reassessments of patient/family needs and provide counseling as required.
  • Offer crisis intervention and individual or family counseling when indicated.
  • Develop and maintain contact with community agencies and services to promote interagency cooperation and facilitate related referrals.
  • Provide ongoing counseling related to issues of death and dying to patients and families as needed.
  • Participate as a member of the Bereavement Team as assigned.
Requirements
  • MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE).
  • One year of experience in Home Health Care preferred.
  • Hospice experience preferred.
  • Valid Driver's License, car insurance, and reliable transportation.
  • Required MI Master's Degree of Social Work.
