Job SummaryWe are seeking a skilled Social Worker (MSW) to join our interdisciplinary team. As a Social Worker, you will provide psychosocial support to patients and their families, utilizing your expertise to identify needs, goals, and interventions. Your primary function will be to assess and address the psychosocial needs of our patients, ensuring they receive the best possible care.
Responsibilities- Provide psychosocial support to patients and their families, including assessment, goal-setting, and intervention planning.
- Collaborate with the interdisciplinary team to develop and implement patient care plans.
- Conduct regular assessments to identify patient needs and update care plans accordingly.
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in ongoing professional development to stay current with best practices and industry developments.
- Master's Degree in Social Work from a CSWE-accredited school.
- Licensure as required by the state in which the hospice is located.
- One year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred.
- Knowledge of terminally ill patients and their families, as well as understanding of hospice and psychosocial dynamics of illness, loss, and death.
