Job Summary

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Functional Medicine Healthcare Provider. This is a full-time position that requires a strong commitment to delivering exceptional patient care and a passion for staying up-to-date with the latest advancements in functional medicine.

Key Responsibilities
  • Conduct comprehensive new patient evaluations, including history and physical exams, and develop personalized treatment plans under the supervision of our medical director.
  • Collaborate with our health coaching and nutrition teams to assess and implement patient nutritional needs and program progress.
  • Provide ongoing patient care, including interpreting functional medicine labs, interacting with patients through our messaging portal, and leading follow-up visits.
  • Attend and participate in morning and afternoon clinical team meetings to stay informed about patient care and treatment plans.
  • Order labs and therapies through our electronic systems and maintain accurate documentation standards.
  • Educate patients about their conditions and treatment options, and provide emotional and spiritual support as needed.
  • Refer patients to external consults when necessary and communicate with their other medical providers as needed.
Requirements
  • Nurse Practitioner or Physician Assistant with a minimum of 3 years of clinical experience post-graduation.
  • Preferred but not required: certification in functional medicine (IFM/A4M/MAPS/SAFM) or comparable experience.
  • Proficiency in using electronic medical records (EMR) systems.
  • High attention to detail and commitment to integrity.
  • Excellent interpersonal skills and ability to make clinical decisions within scope of practice.
  • Dedication to deepening clinical knowledge and staying up-to-date with the latest advancements in functional medicine.
