Job Summary

The Nurse Care Coordinator will plan, organize, and direct hospice care, utilizing their expertise in nursing and community health education. This role involves building on the resources of the community to provide services that meet the needs of individuals and families within their homes and communities.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine hospice needs, providing a thorough physical assessment and history of current and previous illnesses.
  • Provide professional nursing care by utilizing the nursing process, assessing and evaluating patient status, and participating in revising the plan of care as necessary.
  • Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, involving the patient and family in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures, administer medications and treatments as prescribed, and counsel patients and families on meeting nursing and related needs.
  • Provide health care instructions to patients as appropriate, assist with activities of daily living, and facilitate self-sufficiency and optional comfort care.
  • Act as Case Manager, coordinating patient care for assigned caseloads and communicating with community health-related persons to coordinate care plans.
Requirements
  • Graduate of a National League for Nursing accredited school of nursing.
  • Minimum of two years of experience, with at least one year in public health, home care, or hospice nursing preferred.
  • Registered nurse with current licensure to practice professional nursing in the state.
  • Excellent observation, problem-solving, verbal, and written communication skills, as well as nursing skills per competency checklist.
  • Ability to organize and prioritize workload independently, with prolonged or considerable walking or standing, and ability to lift, position, and/or transfer patients.
Additional Requirements
  • Maintain current CPR certification.
  • Ability to drive and/or have access to transportation for agency-related travel.
  • Self-directed and able to work with minimal supervision.
