Millennium Home Care, a trusted leader in the home health industry, is seeking a dynamic and experienced Licensed Practical Nurse (LPN) to join our team on a full-time basis.
Responsibilities:
Deliver exceptional nursing care to patients in the comfort and familiarity of their own homes.
Administer medications and treatments as prescribed by physicians, ensuring the highest standards of patient safety and well-being.
Monitor patient vital signs, assess health status, and communicate any changes or concerns to the healthcare team.
Collaborate with interdisciplinary team members to develop personalized care plans tailored to each patient's unique needs.
Educate patients and their families on effective healthcare management and disease prevention strategies.
Requirements:
Valid and active LPN license in Florida, demonstrating your commitment to excellence in nursing practice.
Minimum of 1 year of experience in home health or a related field, showcasing your expertise in providing quality care in a home setting.
Proficiency in nursing assessments and care planning, allowing you to deliver personalized and patient-centered care.
Excellent communication and interpersonal skills, enabling you to establish rapport and build trusting relationships with patients and their families.
Ability to work independently while also collaborating effectively within a multidisciplinary team.
Valid driver's license and reliable transportation to travel to patients' homes.
