As a Clinical Nurse Specialist, you will serve as a vital resource for nursing staff, guiding the development and execution of care plans for patients with specific needs such as ostomies, draining wounds, pressure ulcers, and incontinence. Your role will be pivotal in promoting patient satisfaction and implementing best practices in nursing care.
**Responsibilities**- Act as the primary educator for patients undergoing ostomy procedures.
- Conduct assessments for patients and their families pre and post-operatively to facilitate adaptation within 72 hours of the procedure.
- Provide comprehensive education on ostomy care to empower patients and their families towards independence and self-management.
- Contribute to discharge planning and referrals based on ongoing assessments of patient conditions.
- Perform pre-operative marking of stoma sites, considering patient anatomy and lifestyle.
- Evaluate the effectiveness of interdisciplinary care plans and recommend modifications to address evolving patient needs.
- Serve as a clinical consultant for managing patients with compromised skin integrity due to various conditions, making regular rounds on nursing units.
- Collaborate with nursing staff to devise care strategies aimed at preventing pressure ulcers in at-risk patients.
- Assist in developing tailored care plans for patients with specific skin integrity issues.
- Monitor patient responses to treatments and collaborate with medical teams to establish treatment goals.
- Deliver ongoing education to staff and patients regarding skin care practices.
- Act as a clinical resource for managing patients with draining wounds or fistulas, providing consultation as needed.
- Work with staff to create care plans that address the characteristics of wounds and drainage.
- Facilitate in-service training for staff on wound management techniques.
- Assess the effectiveness of care through regular evaluations of wound conditions and discussions with medical personnel.
- Identify and address the educational needs of nursing staff.
- Provide insights on new treatments and procedures related to ostomy, wound care, and incontinence.
- Contribute to the formulation of policies regarding wound, ostomy, and incontinence care.
- Participate in the review and enhancement of existing policies to align with current best practices.
- Continuously evaluate ostomy and skin care products, recommending cost-effective options as necessary.
- Engage in research initiatives related to enterostomal nursing practices, including benchmarking activities.
- Demonstrate proficiency in analyzing data to support informed decision-making and performance improvement.
**Education, Knowledge, Skills, and Abilities Required:**
- Valid NJ State Professional Registered Nurse License.
- Graduation from an accredited nursing program with a Bachelor of Science in Nursing.
- Completion of an approved enterostomal nurse program.
- Current board certification in E.T. nursing or eligibility within one year of hire.
- A minimum of five years of experience in medical-surgical nursing.
